Description

More people are living with the late and long-term effects of cancer treatment due to greater prevalence of cancer and increased survivability, especially among aging populations. The specialized healthcare needs stemming from cancer treatment can be most effectively addressed through a survivorship care plan (SCP). However, the effectiveness of current SCPs is limited by their format (typically a paper reference document), lack of easy to use tools for tracking adherence, lack of resources for coordinating survivorship care, and limited access to relevant health measures outside the clinic. We designed and demonstrated a POSTtreatment Health Outcomes of Cancer (POSTHOC) platform that incorporates mobile health (mHealth) technologies and adheres to current health IT standards to manage survivor health through a digital SCP. POSTHOC leverages mobile and wearable technologies to reinforce the SCP by automatically tracking health metrics and adherence to key recommendations and health through a mobile interface. In this way, POSTHOC gives providers tools that extend beyond the walls of the clinic to assimilate and manage patient health outcomes, and empowers survivors to manage their ongoing post-treatment plans and maintain positive health behaviors.
