Description
Empower the lowest ranking individual to submit “sole-source” incidents and near-misses (meaning no one would know if they did not submit it) without fear of retribution so that major problems can be addressed before they happen. Create a database of these incidents and put them into the pockets of the personnel that need it through mobile technology. Finally, encourage users to review incidents through gamification so they can learn from these mistakes and not repeat them, saving lives, limbs, and money.
