TEAM of Care wants to help close the gap between the traditional healthcare model to the new value-based approach by providing its innovative and proven technology to the Air Force Military Health System. TEAM of Care’s software uses a proprietary algorithm to generate, assign, and route interventions in the form of action to clinical and administrative staff. The application includes configurable workflows for Care Coordination, Quality Measure collection and tracking, and Risk-Adjustment Factor management.
