Project Summary Frailty is a common clinical condition in which older adults are at high risk for fallsdisabilityhospitalizationand deathApproximatelyof adultsyears or older are frail andare prefrail with high risk of progressing to frailtyCurrent screening for frailty hinges on infrequent o ce visits and a patient s report of symptomswhich often fail to detect subtle declineImpaired physical function is a major indicator of frailtyHoweverclinical evaluations do not adequately re ect daily life functional performanceIn Phase Iwe demonstrated the feasibility of our HOME system to continuously measure functional mobility fordays in older peopleWe have shown that HOME measures are more sensitive than gait in the clinic to indicate physical health status and future fallsIn this Phase IIwe aim to determine if HOME can better identify geriatric healthfrailtyas well as fall riskWe will prepare HOME for a commercial launch as therst continuous monitoring system to help clinicians and clinical researchers identify frailty status and fall risk and determine the e cacy of interventions on actual functional performance in real lifeOur speci c aims areAIM IClinical ResearchDemonstrate the clinical utility of HOME in identifying frailty status and fall riskAIM IITechnology DevelopmentPrepare HOME for clinical practice and FDAkclearance to clinical marketWe will develop an instrumentedlightweight ankle wrapSmartSoxwith inertial sensors for continuous gait and turning monitoringexpand our Mobility Exchange clinical data management system to facilitate access to mobility data and generate clinical reportsand complete the HOME validation and veri cation
