Description

Non-compressible Torso Hemorrhage (NCTH) is the leading cause of preventable battlefield death for many decades.In recent years, there has been a resurgence of interest in intravascular occlusive technologies as a solution for this persistent cause of death in both civilian and military sectors.The Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) device is the current state of the art.Our technology,the Selective Aortic Balloon Occlusion in Trauma (SABOT) device is the next generation of aortic occlusion which, not only allows selective aortic occlusion and maintenance of flow below the site of injury, but also provides a seamless transition to Selective Aortic Arch Perfusion (SAAP).With a single device, the patient who presents with hemorrhagic shock may be treated with Extracorporeal Life Support (ECLS) by repositioning the device.The design of our study tests the feasibility of bypassing the injured vascular bed to supply uninjured distal vasculature, thereby saving viable organs (i.e. kidney, gut, pelvis and lower extremities.We use two sets of Yorkshire Landrice Swine in a head-to-head comparison with REBOA in a hemorrhagic shock model.Endpoints include: hemodynamic performance, perfusion measures,post occlusion flow and organ failure.
